Susanne Campbell, RN, MS, joined the UMass Medical School's Program Development team in 2013 as a content expert, having assisted primary care practices in a community health center and a hospital outpatient practice achieve NCQA Patient Centered Medical Home Recognition. This “hands on” experience had been a valuable asset as it adds credibility to understanding the work that needs to be done from the practice point of view. Susanne received her Master in Gerontology Nursing from the University of Rochester, her Masters in Health Service Administration from Salve Regina University, and her BSN from Mount Saint Mary College in Newburgh, NY. Susanne has a broad range of clinical and administrative experiences in outpatient primary care practices, community health centers, home care, long term care and providing assisted living services. Her current position is working at UMMS as a Senior Project Director, supporting the expansion of primary care practices for the Rhode Island Care Transformation Collaborative (CTC) Program. She has assisted primary care practices with achieving NCQA recognition and is a Patient Centered Medical Come Certified Content Expert. An important aspect of her work with CTC-RI has been working with nurse care managers, health plans and practice reporting staff in developing a care coordination population health framework for working with high risk and at risk patients.
The mission of the Care Transformation Collaborative is to lead the transformation of primary care in Rhode Island in the context of an integrated health care system; and to improve the quality of care, the patient experience of care, the affordability of care, and the health of the populations we serve.
