CTC Announces Primary Care Practice Sites Selected for the July 2017 Expansion

The CTC Strategic Planning goal seeks to transform 80% of RI primary care practices and clinicians towards practicing in a PCMH by the end of 2019. Following an April 11 "Call for Applications," a CTC Selection Committee reviewed applications and selected practices for this 2017 expansion.
CTC extends our appreciation to the Selection Committee members who participated in reviewing applications and selecting practices.

Neighborhood Health Plan of Rhode Island is in the process of finalizing conditions of participation in a Neighborhood Health Plan contract for a limited number of selected practices. Those practices will be notified of any restrictions that might apply to practice participation in the NHPRI contract once finalized.
CTC will be hosting a new practice orientation session on 7/20/17 from 5:00-7:30pm at HealthCentric Advisors (235 Promenade Street-Suite #500, Providence, RI). During this orientation, CTC will introduce the Management Team and other key stakeholders that support the adult and pediatric practices, and provide information on the service delivery and reporting requirements, committees and practice transformation resources. It is recommended that the provider champion and other key staff members plan to attend the orientation program.
