PCMH Kids: Postpartum Maternal Depression Screening Learning Collaborative Opportunity

A guiding principle of the PCMH-Kids initiative is that patient centered medical homes address the physical, developmental, behavioral, social, emotional, environmental, and oral health needs of children and youth. As such, the Care Transformation Collaborative of RI (CTC-RI), Tuft Health Plan, and the RI State Innovation Model (SIM) have provided funding for PCMH-Kids pilot practices to participate in a behavioral health learning collaborative focused on Maternal Depression.

This learning collaborative will kick-off with an open discussing to hear from the practices on their ADHD project journey.  The second-half of the morning will offer facilitated team time where attendees will sit with their organization to define a problem statement, a unit of measure, and identify action steps in regards to maternal depression.

Participants will leave with a continuous quality improvement project to implement over the course of this learning collaborative. PCMH-Kids pilot practices will receive additional facilitation hours to help them implement these projects and be prepared to present their findings at a final meeting in the coming year. Recommended attendees include no fewer than 3 people from the PCMH-Kids practice (the provider champion, care coordinator, and office manager), and will receive a stipend for full participation in this learning collaborative.

Other interested parties are welcome to join the first half of the morning for information and discussion on the ADHD Learning Collaborative project close out, but will be dismissed during the dedicated team time.
