CTC Announces New Co-Chairs for State-wide Community Health Team Committee
CTC is pleased to announce the appointment of Liz Fortin, LICSW, and Nancy Sutton, MS, RD, as co-chairs for the state-wide CTC Community Health Team (CHT) Committee.
The state-wide CHT has worked to develop a statewide strategy for the development, spread, implementation, and evaluation of community health teams. The state-wide Community Health Team Committee is additionally charged identifying learning needs (including CHT staff and Community Health Workers placed in community settings) and working to bridge those gaps through identification of resources and training opportunities.
About Liz Fortin, LICSW
Program Director of Community Health Teams for CTC, Liz is a healthcare professional with 30+ years of experience in behavioral health and primary care agencies; inpatient and community settings. She graduated from Boston University with her Master of Social Work degree and from RI College with a BA in Psychology. Liz has many years of program development experience which she has applied to the development, implementation and ongoing integration of the community health team within primary care practices and across the health care continuum as part of the leadership team for South County Health. She provides operational and performance oversite for a sister CHT team implemented by Blackstone Valley Community Health Center. The CHT pilot sites began in 2014 and have received funding through a multi-payer health insurance initiative managed by the Care Transformation Collaborative of RI.
Prior to joining South County Health, Liz worked in an FQHC directing the implementation and ongoing management of the agency wide electronic health record, developed the organization's CQI structure and lead a team of staff on successful NCQA PCMH recognition and EHR MU applications. Liz has worked in varied settings as a licensed behavioral health clinician. Her early professional years were in Community Mental Health, working as part of a multidisciplinary team, providing community based case management services to individuals suffering from severe persistent mental illness.
About Nancy Sutton, MS, RD
Nancy Sutton, MS, RD has been the Center Lead of the Center for Chronic Care and Disease Management within the RI Department of Health since September 2016. Ms. Sutton has worked in public health since 1990. Her work has focused on the prevention and management of chronic diseases including cancer, diabetes, hypertension, heart disease, arthritis and asthma. As Center Lead, Ms. Sutton works with programs internal and external to the department to integrate key elements of chronic disease programs into larger system-level initiatives and align programs with the State of RI Integrated Population Health Goals and the Overarching Goals and Strategic Priorities of the RI Department of Health.
