CTC Community Health Team Presents at Massachusetts Department of Health 8th Annual Community Health Worker Conference

The CTC-RI South County Health Community Health Team (CHT) was asked to present it's CHT model at “The Power of Integration” Community Health Worker conference.  Liz Fortin, MSW, LICSW, Director of CHT; Marie Padilla, BS, CPRSS, CHT Team Lead; and Susanne Campbell, RN, MS, CTC Senior Project Director outlined key components of the CTC-RI community health team model that includes:

  • a multi-payer funding approach;
  • builds on existing relationships with primary care practices  and the South County Health System;
  • operates within a geographic region to support patients with complex health needs; and
  • uses key metrics to measure, monitor, and improve performance.
