Seeking 2 full time Nurse Care Managers for our Providence, RI location to join our current team. The NCM will assist in coordination of care for a large, busy and diverse patient population. Applicant should have a strong background in outpatient community health care, prior care management experience preferred. Familiarity with PCMH, NCQA as well as quality improvement benchmarks preferred.

Position is office based with no evenings, weekends or holidays. Candidate should have experience with electronic medical record documentation

Tri-County Community Action Agency

The Community Health Worker (CHW) is responsible for helping clients and their families navigate and access community and healthcare services and adopt healthy behaviors. The CHW supports the medical home model through an integrated approach to care management and community outreach with the goal of improving health outcomes for chronically ill clients.

Experience/Skills Preferred:

Ability to communicate effectively, verbally and in writing;

Ability to operate computers and other office equipment;
Ability to write reports and maintain records;
Ability to handle sensitive and confidential matters with discretion and tact;
Ability to interact with the public, health care providers, and community organizations, and social service agencies;
Ability and willingness to provide emotional support, encouragement and motivation to clients; Confidence to communicate and outreach to other community health organizations and personnel;
